The most-evidence-backed longevity interventions cost nothing. The longest-lived populations on earth — Okinawa, Sardinia, Loma Linda, Nicoya, Ikaria — don't have concierge medicine. They walk, eat plants, sleep well, and stay connected. That's the foundation of every other tier.
What scales with spend isn't intervention quality — Zone 2 training doesn't get better at $20K than at $0, and neither does sleep. What scales is how comprehensively you measure what you're doing, and how much professional help interprets the data. Three strategies, same evidence, different jobs.

Foundation — ~$300/year. What everyone should be doing.
This is not the starter tier. It's the literal foundation every other tier builds on. If you do nothing else from this page, do this — the evidence supporting these interventions is stronger than the evidence behind any premium tool or test.
Total: ~$300/year, mostly covered by what you're already paying for through insurance. The only out-of-pocket spend is the Quest blood panel (~$150) and the cardiac calcium score (~$150 one-time if you're over 40 and not in a low-risk profile). Everything else is free protocols, insurance-covered screenings, and a one-time $80 chest strap.
The intervention layer is the heaviest-lifting part of any longevity plan. Higher tiers add real but smaller marginal returns. Get the Foundation right first.
Measured — ~$1,500–3,000 first year, ~$1,200/year after. Foundation plus best-in-class consumer data.
For the engaged self-manager who's already doing the Foundation and wants to know whether it's working — with the resolution that comes from continuous tracking and comprehensive testing.
Total first year: ~$3,000 (the Prenuvo is one-time). Steady state: ~$1,200/year. This is the sweet spot for the high-income professional who isn't ready to hire a medical team but wants the data to make smart decisions.
Concierge — $15,000–25,000/year. Hand it to a team.
For the person whose constraint is time, not money. Concierge longevity practices handle the assembly, the testing schedule, the result interpretation, the physician oversight, and the ongoing adjustments. You show up; they handle the orchestration.
Concierge is for people who value their time enough that paying $20K to delegate the assembly is worth it. It's not a class signal — it's a job signal. The intervention quality is the same as Measured.
An honest note
We're publishing this tier sparse on purpose. Fountain Life is the most-established national option we'd point to today. Mayo Executive Health is a defensible alternative if you can travel to Rochester and want a more conservative, evidence-based posture. Beyond that, the concierge longevity space is evolving fast — we're evaluating regional practices and will add named recommendations as we verify them.
A note on mixing tiers
Most readers won't be purely one tier. A Foundation-strategy reader who wants to splurge on Oura can — click through to the wearables comparison and pick the Premium or Value pick there. A Measured-strategy reader who's skeptical of full-body MRI can skip that row. Each row in the grid links to its full comparison article. The grid is the executive summary. The articles are the depth.
